What gets me about these blinded studies for cohorts at higher risk of contracting hiv is the inherent contradiction to the study itself. It represents something of a faux ethics for the practitioners of biological and medical sciences. Ultimately if you are studying msm who practice high risk sexual activities to test the efficacy of a prep or pep then don't you recruit them exactly on the basis of their high risk activities! It defeats the purpose to give them extensive harm reduction counselling to reduce risk-taking behaviour because it may well be this and not the prep at all that reduces the infection rates in the resulting data - rendering that data negligible in its usefulness at best!

I'm aware of a study that indicated that PrEP decreased the incidence of high-risk behavior among MSM, but the researchers were not sure why. I think PrEP is a heartening development though I can't help but wonder if in the long-run, PrEP will enhance the attitude of "I'll do what I want, to whom I want to." People aren't afraid of the virus like those of us who lived through the 80's and early 90's. The fear seems to have been replaced by the notion that taking a pill will make it as if nothing has happened. Some of my clients take 29 pills a day... I am sort of thinking out loud here, but I really want to know how to wrap my head around this.
